Our membership plans were created so that you can be in control and customize your dental care, all while receiving a high standard of care in a comfortable, home-like atmosphere.
 
 
 

Our membership plan is not insurance, but a discount plan. It cannot be used along with any other dental plan, workers’ comp claims, referrals to specialists, insurance claims, or in conjunction with other discounts or promotions. Coverage is in effect for one calendar year beginning on membership enrollment date with membership fees paid in full. Appointment times are limited. It is the responsibility of the member to schedule appropriate visits. Any dental procedure in progress or completed before or after a member's eligibility period is excluded. Patients who do not use all plan benefits within one year may be eligible for money back. If requested, there will be an administrative fee of $99.
